Correction of name and D.O.B

Sir,

My voter card has some mistakes i have corrected it by submitting FORM 8 with appropriate documents to municipal corporation but still their is no correction in my voter card. So, please take any action for correcting it.

Below i submitted my file of application through online process of correction.
