There are two wrong entries. Pl. correct them and send the new card to the Adrress registered.

1.The DOB printed is 08/01/1950.

It is to be corrected as 08/01/1951.

2. Father's Name printed is 'Sathaiah Bonthala'

Pl. correct it as'Sathyanarayana Bonthala'

Pl. send the card after correction to this address.
