Returned & Refun of 1 Touch Device

This has reference to my several communications, written and verbal (through Call centre; Bank official) but all in vain. I am last reminding you to credit my aforesaid Savings Bank account with an amount of Rs. 800.00 so charged while issuing One Touch Device bearing SL NO 0780111640/ BN: 0780022. The device was returned duly on 04/11 to the branch and acceptance received under seal and signature (copy attached).

The request was put forth for Net Secure Authentication while doing transaction through Axis Bank web portal, which is free of charge, whereas, the Branch has issued One Touch Device and charged an amount of Rs. 800.00 (Rupees Eight Hundred only). I feel it is misunderstanding at the branch level. Moreover, the branch is sleeping even after return of the device and not reversing the amount so debited (copy attached highlighting the transaction).

This is my earnest request to rectify credit my Saving Bank A/C with an amount of Rs. 800.00 Interest for 19 months to saving account and register myself for NET SECURE ENABELLED TRANSACTION. My Mobile no 9971014181 shall be updated for NETSECURE PASSWORD.
