Regarding Non availibility of New meter connection Form

This is Paras Bindal From the residence of Champa Chhatisgarh. I want one 3 phase metre connection. I had gone many times electric office to purchase a metre form but the employes of office always telling that the form will available with in a week. I had waited From one month for that new metre connection Form and it has not available to me yet now.
