Amount debited without access

I am an HDFC bank customer, i have an salary account with HDFC bank..

on 22 July, 10, 000 rupees were debited from my account and i never used my card and has not swiped my card anywhere, then how come the card shows 10, 000 rupees withdrawn from the ATM.

this is a major mistake from HDFC side.
