Delay in renewing my policy

My Health policy named Hospicare -Policy no.11366610 was given for renewal after 3 months laps period.accordingly I have paid my premium along with the self declared medical form on 17th morning of January. As I have not received the policy renewal confirmation of my policy within 15 days of receipt of my payment, I have contacted Your Kozhikode Office. Subsequently I received a SMS mentioning that I should go for a Medical check up for which contact the nearest ICICI office.

I would like to know why the delay and who is responsible for this lapse.
