Hello

I, Tejas Jhaveri, a ICICI cc holder since more than 5 yrs now, CC number xxxx xxxx xxxx xxxx, would like to inform that there has been some miscommunication between bank and me. I had received cc statement for the month of sep '10 after the due date, your record shows that it was received before the due date but it is without the receiver signature that proves that it was not delivered to me. secondly when i received the fresh copy i made the payments. later next months statement was not delivered on my address again. it was sent to some other persons address and that person sent me a notification that my statement was sent to his address and asked me to pick it up from his address. Thus this was the second time.

After all this mistakes form bank's dispatch department they say they had sent me an email and also phone notification, but i dont have email and phone access due to my working conditions, where both are prohibited. thus only means of communication for me is hard paper, and the same is important for me as only if i submit my hard copy statement the payment form accounts section is done.

Now after being an old customer and a reasonable user i request you to sort this matter and get it resolved.
