Payment of annuties (Policy No. 482101004)

Hello Sir,

I do hereby inform you that I have got Jeevan Suraksha policy No.482101004. From November to till date I had not received any annuities in my SBI Account (A/c No. 10432963228). I had already send 2 (two) speed post letter of Certificate of Existence, NEFT Mandate form, etc duly signed and attested by bank on 09/09 [SP EU835639878IN] to the A.O. (J.A./J.D.Cell), LIC of India, North Central Zonal Officer, “Jeevan Vikas” 16/98 M.G.Road, Kanpur-208001 and another letter on 23/04 [SP EU882128569IN] to the LIC of India, KMDO-1, Jeevan Suraksha Unit, ‘Jeevan Prakesh’ 2nd Floor, 16 CR Avenue, Kolkata-700072, but till now I had not received any annuities or letter from your Office regarding this matter. I think that your Office will consider this matter seriously.
