Issue of Duplicate Pass Book & allowing for payment of premiums w.e.f. 01.10.2009

I have lost my PLI Pass Book (Policy NO.MP-40485-US) during October & complaint lodged verbally many times with locak post office (Deepanjalinagar, NTPC-Simhadri, VSP) and wrote letters during Feb.10/March, 10 & on 30.06.2010. Till now I have not received any communication and not allowed to pay my long outstanding premium amounts @ Rs.149/- per month. I have contacted Anakapalli Post office and I have been informed that If I didn't pay continuously for a period of 11 months or more, my policy will be lapsed. Hence, I request you to do the needful for continuity of my PLI & issue of duplicate pass book.
