This is in regards to my Hospital Conveyance Allowance that i have not been receiving since 15-January

Dear Sir,

This is in regards to my Hospital Conveyance Allowance that i have not been receiving since 15-January.

I have already submitted all the applications in regards to Hospital Conveyance Allowance to, State Bank Of India, Innespeta Branch -1013, Rajahmundry 533101.

I have sent the Hospital Conveyance allowance applications through SBI Four times but there was no positive response at all.

The applications were sent on 03-03, 05-07, 16-06 and 06-08.

The applications were sent to the below address,
