Not recived policy document

I have purchase a car insurance policy (car no. DL3C AX3548) from your company in 11/01.But at the time of Insurance the guy who came to Insure my car he only gave me the cover note (150100687-17168) and took the Insured amount cheque (163103 of Canara Bank) of Rs.4670 which was debited from my account. But till date i haven't received the original insured document from your company. Please provide me the original document ASAP.

Note:-I have a conversation with Mr. Manoj (Employee of National Insurance company of Govindpuri Branch) who told me that your car is not insured till date. So kindly have a look over this issue and resolve it as soon as possible before any problem occours.
