Purchased lic health plus in the name of sudhanshu kumar, jyoty kumari & sachika on 24th march from lic samastipur branch.policy no. 536391279. jyoty kumari got hospitalised on 24th april to 5th may.it was informed to lic agent as well as lic branch samastipur.policy document not received till date.it was found that sachika name (child) not incorporated in policy.premium due on 24th march. please help.
