Refund the money

I ve applied a broadband connection in airtel through a airtel executive (Name: SARAVANAN Mob. No.9790934006) on 11/07. I gave him one passport size photo, address proof, and Rs.1000 cash. He gave me a customer enrollment form (CEF No: CH 1221075) n said that i ll get broadband connection in 5 days. After 5 days i called his no. but it was in not reachable till today. So kindly do the need full to solve this problem.
