Regarding ADD ON CARD

Hello Sir

i have applied for ADD On card around one month back twice .once thru bookpost and 2nd time thru Blaze Flash Courier... along with tht i also requested for credit limit enhancement but till now i havent got add on card nor i got any enquiry... when i enuire in customer care they said they havent recieved the form...

so wht shud i do now...
