Refund of P.F. amount

Dear Sir,

I am having a P.F. account No. MH/45434/5717 with EPF of India, Kandivali (W). I applied for my PF refund on 12th Sept. but till date (22nd Nov. 2008) I didn't get neither my amount nor any information about it. If you could help me to find about status of my application.
