Deduction of inst.eventhough loan has been fully paid

I have allready paid up my icicibank homeloan in aug-08 and till this running month bank debited 125.00 Rs.every 10 th date of the month against LBHNR00001526751-LN INAST. 3 times i have informed to the bank to stop this debiting but till today it is not cleared. So now what i have to do please guide me. My ICICI bank saving A/c no. is 045201500666
