LIC's Life Insurance Policy
Cheque not delivered, Bond not received

Dear Sir

I have the following complaints, though I have inquired several times, no reply got yet

1) I have not received my Policy Bond against my policy no 940472430 (Now at Puri-Orissa, Branch but bought at Satara-Maharashtra)

2) I have not received the money back of Rs5000/- since Yr 2005.

Please look into the matter.
