CLOSING OF LIC MONEY BACK POLICY AND NON PAYMENT OF FIRST SB AMOUNT AFTER COMPLETION OF 4 YRS

Dear Sir,

1. I Mr RADHA MADHAB MOHANTA serving in defence have a JEEVAN SURABHI MONEY BACK POLICY NO - 970840786 opened on 22 Oct 1997 for 15 yrs plan from NAGPUR which had lost during the year 2001 in train journey and later I had approached to your good office through your Agent namely MR MG HARGUNANI. He has helped out me all about the policy and also revived after making payment of Rs 3657/- through DD. But till date no any correspondence received either from your good office or from above named Agent. I had applied several times to your office either for closing my policy and for renewal the same.

2. Sir, keeping in view of the above, may I request you to kindly close my above policy and refund all my premium amounts whatever deposited by me from Oct 1997 after deducting your all maintenance and other charges. Because, now I have no interest with such delay response in your policy. Please do the needful.
