Charged twice-compliant

I have been charged twice on 15th of Aug'08 . i made call to customer care and they called me and queried about the same but still it reflects in the statements i dont know how to solve this. kindly go through details

With out my signature in the charge slip the bank realeased the amount to the merchant.
