PAN card correction

Dear Sir,

This is to bring to your kind notice that i had submitted a request for correction of my name and DOB in my PAN card on 21.08.2009. I have not received any further intimation on the correction being made and no one has ever contacted me for any further procedures needed.

Its been more than a month and i request you to kindly look into the matter and approve the correction in my PAN card at the earliest.
