Non receipt of pass port

Sir most respect fully i beg to say that this is afzal husain from moradabad [u.p].

I submitted my p.p form on 20/01/09 in moradabad office and my file no is z160064/09 but now i have not received my pass port i submitted two times my compelet documents in bareilly office first on 25/06/09 and then second time on 12/07/09.sir please issue my pass port soon
