Sim card deactivated without notice

I purchased airtel prepaid sim card recently at Chennai by submitting all the requirements and completed all formalities in one go at the retail store. I was given sim card and recharged for Rs. 120/- and south pack top up for Rs. 27/- . After ten days my sim was disconnected without any notice saying that pending documentation formalities/verification etc., Then I went to the airtel store at IInd line beach, chennai and enquired about the formalities to be completed, to my surprise, I was told that the said sim card was in the name of Kumaresan. either retail seller or airtel store could not solve the problem. Hence, i request the concerned airtel telecommunication authorities to solve the problem and activate the sim to enable me to use the services.
