Non receipt of ceque ook even after repeated requests

I have not recieved a cheque book from the bank even though i have sent the requisition form in Nov and thereafter sent two reminders to the bank. On getting no response i approached the local branch in delhi who also sent a couple of e mails to the bank and also spoke to them on the phone.still the bank is not responding.i do not undrstand why this ineffeciency and gross neglegent attitude of the bank??? pl help
