I have applied in the name of AKASH MAHESH KALGHATGI 500 units for CGGF, form no-745611 deposited on 20.10.1995 at DENA BANK UTI Extension counter. The amount has already been debited from the account. I have still not received the certificate for the same. Kindly send me a duplicate copy.
