Bajaj alliance capital unit gain

Dear sir/Madam,

I have bajaj alliance life insurance in capital unit gain policy. I completed three installment of 10000 Rs. each. Statement of two installment are not received or not sent by yr end.

Till date i have no knowledge of No. of units in my account and Allocation of units in diff. funds.
