Dear Madam/Sir,

Today my mother Mrs. Madhuri Narayan Cheulkar (Age 62) went to Bank of Maharashtra, Kalyan Branch for submitting life form of my father Mr. Narayan Rajaram Cheulkar (Age 67) who is bedridden since 2006.

After submitting form Bank's staff told my mother that they need extra signature of Doctor where my father used thumb as a signature. But there is nowhere mentioned in the form about this extra signature. Also Doctor has provided certificate regarding patient's condition and his unavailability.

Then why this staff is asking for the signature of Doctor?

Why they are not providing complete information to customer so that old people will face less problem attending bank for transaction?

Please do the needful.
