Name of Pallav Pathak, s/o Virendra Pathak, H No. 537D / 65, Srinagar colony, mohibullapur, Sitapur road, Lucknow - 226021, Dob- 01/07/1989, submitted by application on 10/08/08 for inclusion of my name in the voter list. Along with application I submitted necessary address proof & photograph also. Till date I did not get voter id card. I also could not find my name in the voter list of area - 237, 236 & 238.
