Maturity payent cheque lost by my banker, Request for re-issue of cheque to me

Sub:- Re-issue of maturity payment cheque/draft in respect of my money back policy No.540402623 . The received cheque was deposited in my bank for clearance, but lost by bank.

Dear Sir / Madam,

I had deposited the cheque for maturity payment of my above policy at my UNITED BANK OF INDIA (JEC TALCHER), Talcher, P.O. Dera Dist. ANGUL, Orissa Pin 759103 (in my savings bank account No. 1722), on 29/03 .

But the said bank has lost this cheque and till date the cheque has not been cleared by my above bank.

My bank manager has written a letter to LIC, Dhanbad for re-issue of cheque to me.

You are requested to kindly re-issue the cheque (amount Rs. 64, 300/-). to me at my above bank account.
