Pancard correction of name

Sir,

I am having permanent account number as "AHIPK1266K" which is being used for tax purposes. In the card instead of my middle name "VENKATA" my father's name "VEERARAGHAVA" is printed, which is to be corrected. Hence I request you to guide me to whom I have to send my request and procedure if any.
