Loss of refund vouche

I had purchased one shirt on 26.01.2008 at around 9pm

at john miller counter from your retail showroom shri vallabh shopping mall s, v.road Borivali (W) Mumbai

which was returned immeditely at counter as it was

defective. One refund voucher was given to take another

shirt in replacement. Due to over croud in shop the voucher was misplaced in shop. I immediately informed

at cash counter to block the refund voucher & they blocked the voucher & asked me to come after 2-3 days to get replacement. I have approached several times but

not getting correct response. Pl.get replacement immediately.
