I filed claim petition along with necessary papers i.e.death certificateissued by competant authority in Form no.6, death certificate issued by SRI RAM HOSPITAL kankarbagh, patna, Money recept given by the hospital., Treament details etc.the branch manager demanded filled in form B and B-1 through his lettre no.21 dated06/11/08.A reply was sent to him vide Regd letter n.1724 dated17/11/08.Lateron Manager claims sent aletterno, BDO/CLAIMAdt07/03 asking to submit original matriculation certificate ot the deaceased shail sinha wife of claiment and duly executed FormB and B-1through branch. The original Matriculation certificate and reply was send vide REGD LETTER NO.4682DT 11/04/09.rEQUEST EARLY SETTLEMENT OF CLAIM
