Non receipt of pf statement

Dear Sir

I am an employ of erstwhile Air Deccan since 4-12-06. presently kingfisher airlines ltd. As on date i do not know my pf balance and the contribution details.

please also let me know the procedure to withdraw or take loans against the balance
