For preperation of SAMRT CARD RC
Non receipt of my RC book

Dear sir,

With ref. to above i hereby submitted the origionel document of my vehicle mh 15 CF 4520, for TRANSFFER at R.T.O. Nasik for on my name in the month of Dec. Which Receipt for Smart Card, Dated 23.12.11 for preperation of Smart optical Vehicle RegistrationCirtificate, is with me. But after the passing of three month the smart card is NOT RECEIVED by me, till the date.
